I hereby certify that all information submitted in this application is true, accurate, and complete to the best of my knowledge. I understand that any false or misleading information may result in denial or revocation of membership.

If approved for membership in the Congress of Christian Leadership, I agree to uphold the values, standards, and governing policies of the organization and to conduct myself in a manner consistent with the principles of Christian leadership and service.

I acknowledge that full membership status and privileges are contingent upon payment of the required membership fee. If such payment is not received, I understand that I will be placed in the Fellowship Tier and will not receive full membership benefits until all required dues are paid in full.

I further acknowledge that the application fee is nonrefundable and, upon acceptance, will be applied toward my first year’s membership dues.
